Requests for laboratory testing

Make sure you give the patient a fully completed application form.
This form should contain:

  • Full name (surname and initials)
  • Date of birth
  • Address
  • Name of the applicant
  • Desired determinations and any clinical data
  • If applicable: the name of the recipient with date and time of collection

A patient sticker is preferably used on the application form.

Acceptance criteria

In order to process material correctly, we ask you to pay attention to:

  • Correct collection material (see: collection tube order)
  • Sufficient volume
  • Correct identification (name and date of birth)
  • Correct sample container (tube/jar)
  • No leaking or contaminated containers
  • For microbiology: no sterile material older than 48 hours
  • We cannot process the material without a request form

Additional provisions

If desired, additional tests can be performed on previously collected material, provided that:

  • There is sufficient residual material
  • The storage period and conditions allow this

Requests outside office hours

Clinical Chemistry

For requests outside 07:00 – 15:30 that do not fall under the Cito package, prior approval from the clinical chemist is required.

Medical Microbiology

For requests outside office hours, consultation with the on-call medical microbiologist is required.

Cito requests

For urgent requests (Cito):

  • Clearly state on the application form that it concerns a Cito.
  • Only determinations within the Cito package are processed urgently.
  • Results are available within 60 minutes after receipt of the material.
  • The result is faxed and communicated to the applicant by telephone.
